Nominations for the 2025 40 under 40 leaders in health awards are open. All applications must be submitted by March 7, 2025 at 11:59pm EST. Awardees will be announced in March and will receive their awards at the NMQF Leadership Summit in April.

A unique aspect of the National Minority Quality Forum’s overall efforts to improve self-care, prevention, treatment, management, and monitoring has been the use of the Forum database to map health data in the United States. Forum indexes and atlases map disease incidence and prevalence, using zip-code-level data.
